Chronic Care Management
Our Chronic Care Management department assists Medicare patients living with two or more chronic conditions in navigating their medical needs.
Chronic Care Management is a brand-new program designed by Medicare to help keep you healthy in between visits. Once enrolled, you will be assigned a qualified and experienced Care Coordinator who will call you at home to help refill medication, schedule appointments, and facilitate other aspects of your care like transportation and community resources. Think of us as your personal healthcare concierge.
Patients are encouraged to take advantage of free health monitoring devices designed for conditions like diabetes and hypertension, which allow your doctor to track your health status without you leaving your home. If patients’ readings are out of the normal range, their care team will be alerted and patients will get the care they need.

Enroll at no or low cost. Medicare will cover a majority of the cost and any additional coverage will be billed to your secondary insurance.
First Choice Health Centers serves patients of all ages and backgrounds regardless of insurance status or ability to pay. We accept most insurance plans and offer a sliding fee scale for patients who are uninsured or underinsured.
To be eligible for a Chronic Care Management program, you must have a provider affiliated with First Choice Health Centers. All care managers work in cooperation with your physician to provide education and guidance to manage your chronic conditions. If you do not already have a physician or nurse practitioner, our team can assist you in selecting one.
