SLIDING-FEE DISCOUNT PROGRAM

First Choice Health Centers also offers a sliding-fee discount program for income eligible patients at or below 200% of the Federal Poverty Guidelines.

2023 Sliding Fee/Federal Poverty Guidelines

Who is eligible?

A patient must provide proof of their income and household size at time of visit to be eligible for the program. The amount of the discount will be based on the patient’s family size and income. The Federal Poverty Guidelines and the Sliding-Fee Discount Schedule determines the patient’s discount.

What to expect when using the Sliding-Fee Discount Program:

When you call to make an appointment, you will be asked to bring:

  • Your insurance card
  • At least a $20 payment
  • Proof of income (W-2s, Individual Tax Return Form 1040, copies of unemployment checks, last two pay stubs). The proof you supply will be used to calculate your discount.

Payment plans:

The Center offers the option to set-up payment arrangements for any outstanding balance. A payment plan can be arranged through an Automated Clearing House (ACH) in which payment will be withdrawn on a select day of the month until your balance is paid off.
